Insurance and Billing

Physicians with the Fox Family Practice participate in most insurance plans, as an in-network provider for numerous PPO's and HMO's. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office or check with your insurance carrier prior to your appointment. Our staff will verify and explain your benefits before treatment, at no charge. If your doctor does not accept your insurance plan, the bill will be your responsibility. If your plan does not cover the services provided, you may be billed for these services. Insurance companies dictate that all co-pays are due at the time of service.

No Insurance

If you have no insurance or do not have healthcare benefits there is still a way for you to receive the care you need. For patients who have little or no health insurance coverage, self-pay discounts and flexible payment programs are available. Call for specific details.

Payments

As a courtesy, we will bill the insurance carrier on your behalf. If you are a member of an HMO, PPO, or other "managed" health plan, you must present your insurance card and co-pay at each office visit After your insurance carrier processes your claim, we will bill you for the portion of the balance that is the patient's responsibility (examples: co-insurance, deductibles, non-covered services). Payment for healthcare services is required at the time service is rendered. Any other arrangements for reimbursement should be discussed with our billing staff.

Late/Unpaid Balances

There is a $35 late fee added to all unpaid balances over 90 days. Additionally, there is a service fee of $22 for any checks that are returned due to insufficient funds. Any questions or complaints pertaining to these billing practices should be directed to our billing office.
